Registration and Records Office Forms

Printable and online forms are available for student use. Several forms must be obtained from the Registration and Records Office due to security reasons.

Academic Clemency Request Form(print version only)
Under specific circumstances, students may seek academic of forgiveness for certain courses. Please see an academic advisor.

Admissions Application (print version/online access)
If you plan to take a credit class you'll need to apply to the college at www.frederick.edu. Once you have been admitted, you may need to take assessment tests depending on your circumstances before meeting with an advisor or enrolling in credit classes.

Appeal-Tuition (print version only)
Use this form if you want to request partial refund of tuition and fees. Please submit completed form to the Records and Registration Center, Building J (fax/mail accepted).

Appeal- Academic Record (print version only)
Use this form if you would like to request a late withdraw, change to audit on your transcript or withdraw after the normal refund period. Please submit completed form to the Records and Registration Center, Building J (fax/mail accepted).

Change of Address (print version only)
Remember, without a correct address on file, you will not be able to receive important notices, including your bill. Please submit completed form to the Records and Registration Center, Building J (fax/mail accepted).

Change of Schedule (print version/online access)
You can update your schedule via the online Student System or you can complete this form if you have already enrolled in classes and wish to change your current schedule. Please submit completed form to the Records and Registration Center, Building J (fax/mail not accepted).

Change of Major (print version only)
Use this form to change your major or catalog year for degree requirements. We recommend you meet with an advisor before submitting your request. If you transferred credit into FCC from other colleges and/or the military, make sure you check the appropriate blank on the form. Please submit completed form to the Records and Registration Center, Building J (fax/mail accepted).

Change of Name (print version only)
Use this form if your name has changed, and provide the Records and Registration Center with official documentation. Please submit completed form to the Records and Registration Center, Building J (fax/mail accepted).

Dual Enrollment PIHE Application(print version only)
Based on senate bill SB740 - Use this form if you are a current FCPS student taking an FCC course. This form needs to be signed off by HS counselor, once approved bring in to the Student Accounts Office located in "J" enrollment services building to get your tuition rate adjusted.

Graduation Application (print version only)
Use this form to apply for graduation if you anticipate completing the requirements for an associate's degree or certificate. The deadlines for submitting the application are published in the academic schedules. Once your application is received, an evaluation will be completed and a status letter mailed. Please submit completed form to the Records and Registration Center, Building J (fax/mail accepted).

Graduation Letter of Recognition Application (print version only)
Use this form to apply for graduation if you anticipate completing the requirements for a Letter of Recognition. The deadlines for submitting the application are published in the academic schedules. Once your application is received, an evaluation will be completed and a status letter mailed. Please submit completed form to the Records and Registration Center, Building J (fax/mail accepted).

In-County Tuition Rate Agreement
This agreement entities employees of Frederick County businesses in some cases to benefit from in-county tuition. Contact the Student Accounts office for more details at 301.846.2456.

Use this form if you are a current or former FCC student and wish to enroll in a course(s) at another institution for transfer back to FCC as part of your degree program. Please submit completed form to the Records and Registration Center, Building J (fax/mail accepted).

If you’ve attended the college since 2003 you can access/print your unofficial transcript via the online Student System or you can complete the form. Regular FCC Transcripts take a minimum of 3 business days to process. FEMA transcripts take between 4 to 6 days to process. Please submit the completed form to the Records and Registration Center, Building A (fax/mail accepted).
